ATK's medical plan options all cover preventive exams, immunizations, and hearing and vision checks at 100%. Other features of ATK's medical plans:

Two dental plan options
ATK employees choose from two comprehensive dental coverage options. Diagnostic and preventive services are covered at 100% with no deductible under both plan options. The plans also feature:
Coverage provided for basic and major restorative services, prosthetics, repairs, and sealants
An optional orthodontic benefit in one plan option
Generally, dependent coverage to age 26; student status not required
401(k)
Participation via automatic enrollment at 6% pre-tax contribution rate, or may generally elect to contribute from 1% to 50% via pre-tax, after-tax, and/or Roth 401(k) basis
Match of 100% on first 3% and 50% on next 3% of pre-tax and/or Roth 401(k) contributions, subject to a one-year vesting requirement
Non-elective (automatic) contribution based on employees age and service, subject to end-of-year service requirement and three-year vesting
More than 25 investment options to choose from
Paid time off (PTO)
ATK-paid personal, vacation, and sick days are combined into this benefit
Annual accrual is based on years of service
Full-time employees can elect to buy or sell up to 40 hours of PTO each year
There are separate, ATK-paid benefits for bereavement, jury duty, and military leave
Flexible spending accounts
Offset your eligible expenses for medical, dental, dependent care, and qualified transportation expenses by setting aside pretax dollars to pay for these costs
Availability of convenient, no-cost debit card for the medical flexible spending account
Basic life insurance
ATK pays for a benefit of 1.5 times annual pay up to a $1 million maximum
ATK provided — no employee contribution or enrollment required on your part
Convertible — you can elect to take coverage with you if you leave ATK
Buy-up life insurance
If elected, additional coverage of one to six times annual pay is fully paid by the employee
Employees who do not use tobacco products pay lower rates for optional, employee supplemental life insurance
Combined total coverage maximum (basic and supplemental) $3 million
Evidence of insurability required each time you request an amount of more than $750,000
Dependent life insurance
If elected, employee pays full cost of this optional benefit
Proof of good health requirement if enrollment not made within 30 days of hire
Spouse coverage options of $25,000, $50,000, $75,000, or $100,000; child coverage options of $5,000 or $10,000
Convertible — you can elect to take coverage with you if you leave ATK
Generally, dependents eligible to age 19; age 23 for full-time students
Accidental death and dismemberment (AD&D) insurance
If elected, you pay the full cost of this optional benefit
Employee elects from one to five times annual base pay, up to $2 million
Spouse and dependent coverage options
No proof of good health requirement
Short-term disability
Basic, ATK-provided benefit of 80% of annual salary, up to 26 weeks
Optional buy-up coverage from 80% of the basic benefit to 100%, up to 26 weeks
Long-term disability
Basic, ATK-provided benefit of 50% of annual salary up to two years for own occupation; age 65 for any occupation
Optional, buy-up benefit covers from the 50% basic benefit to 65%
You pay full cost of buy-up benefit, if elected; enrollment within 30 days of hire does not require proof of good health
Long-term care insurance
Optional, payroll-deduction access to discounted group rates
Covers custodial nursing care not covered by Medicare or health insurance if you become unable to care for yourself due to an illness or injury

Benefit programs listed represent offerings currently available for continental, U.S.-based employees. Plans and programs for employees who work outside the continental U.S. or for ATK’s wholly owned subsidiaries, including Eagle Industries, will vary.
ATK reserves the right to change, amend, or terminate any of its benefit plans at any time in its sole discretion, including the right to change retiree benefits after the person has retired.
